What is the GM Health and Care Intelligence Hub?

For the first time ever we have linked care records across acute, community, mental health, adult social care and now primary care. This unique data architecture is transforming the way we understand health inequalities and access, experience and outcomes of care.   


The aims of the Fairer Health for All portal are to:

  • provide a single repository for all health and care system data, dashboards, resources, and intelligence relating to health inequalities into one place.
  • support a collective and comprehensive understanding of health inequalities across GM by providing good quality robust data and insights and analysis that help us understand
    • Which communities (geographical and communities of interest/protected characteristics) have poor health
    • How we are improving health outcomes for those communities


This will support the reporting and assurance requirements around action to tackle health inequalities at neighbourhood, locality, system and provider level and enable all partners to:

  • Bring data to life, understanding how health inequalities and variations in care change throughout a person’s life
  • track progress and evaluation of plans for tackling health inequalities over time.
  • inform the targeting of work and resources into communities with the highest risk of morbidity or mortality. (aligning with the principle of proportionate universalism).
  • shift focus to addressing multimorbidity, rather than single disease pathways.


VCFSE Intelligence

Voluntary, community and faith sector organisations (formal and informal) collect a range of data and insight that helps us understand inequalities in health for different communities.


In 2025, NHS GM, 10GM,  Local Authority Public Health teams and a range of VCFSE partners will be learning together about how to support cardiovascular disease and diabetes prevention by:

  • Developing skills and capacity in the VCFSE sector for data management and analysis
  • Testing out how combining VCFSE and public sector data can help us capture the insight and stories of change from different communities and share wisdom and learning about what works to keep people ‘well’ and why. 


Through this work we will look at what VCFSE data and insight would help us understand:

  • The assets, strengths and needs in different communities that help people to ‘live well’ and prevent ill-health in Greater Manchester
  • The profile of people accessing different types of support and their patient journey
  • which communities have fewer opportunities to live healthily and are more likely to develop poor health
  • The barriers and enablers to accessing care including:
  • community-led health and wellbeing support, activities and information
  • Screening services
  • primary care- specifically GPs and pharmacy
  • Specialist care
  • Different experiences and outcomes of care
  • The positive and unique impact of the VCFSE sector offer on people's health and wellbeing
  • Understand the skills and capacity of using data well in the VCFSE sector.
  • Referrals into, and between, different types of VCFSE organisations
