CORE20PLUS5

CORE20PLUS5 is an NHS England framework designed to reduce healthcare inequalities by focusing on the most deprived 20% of the population (CORE20), plus other high-risk groups, and addressing five key clinical areas to improve health outcomes (PLUS5). Equality and Inclusion

Equality and inclusion in population health ensures that everyone, regardless of background or circumstances, has fair access to healthcare, resources and opportunities to achieve their best possible health outcomes. Making Smoking History

In Greater Manchester we are on our way to delivering a smokefree city-region. There has been some excellent local delivery in recent years within the boroughs across our city-region, where overall smoking prevalence rates has fallen from 18.4% in 2016 to 12.5% in 2023.

Prevention & Proactive Care

Prevention and proactive care focuses on keeping people healthy by addressing risk factors early, promoting well-being, and ensuring timely interventions to help people live well and independently for longer, especially those with multiple long-term conditions.
